In addition to Home Health Aides, Skilled Nursing and Emergency Response Systems, we are often asked to provide specific patient services. This includes the following:



Statewide Respite Care

Home care is often needed by families on a temporary basis to provide assistance and relief to the primary caregiver. This could be for several hours a day, or for a long weekend.

Services may include:

  • 24 hour live-in help
  • Private duty nurses
  • Adult day care
  • Temporary assisted living placement


Hours/availability: 7 days a week

Patient Cost: Sliding scale, $4.50 maximum co-pay per hour

Eligibility: Recipient monthly income below $1,911 ($3822 for a couple) and liquid assets below $40,000  ($60,000 for a couple)

Community Respite Care

Home care is often needed by families on a temporary basis to provide assistance and relief to the primary caregiver. This could be for several hours a day, or for a long weekend.

Service includes: Home Health Aide Services
Hours/availability: Monday through Friday

Patient Cost: Sliding scale, $4.25 maximum co-pay per hour

Eligibility: Recipient monthly income below $1,911 and liquid assets below $40,000.

Must have one of the following diagnoses: Alzheimer's, Dementia, CVA, Parkinson's, OBS

Health Risk Assessment Program

This program is designed to promote wellness by identifying and addressing health problems early. It includes a comprehensive assessment in the home by an RN.

Services may include:

  • Medication management
  • Case management
  • Nutritional counseling
  • Community resource referral
  • Follow-up by RN and/or
        social worker


Hours/availability: 8 AM - 5 PM By appointment

Patient Cost: No fee, Passaic County Office on Aging grant. Donations accepted.

Eligibility: 60+ years of age, Passaic County resident

Geriatric Care Management

This includes comprehensive assessment of present and future life planning, monitoring, delivery of direct services, and on-going care management activities.

Patient Cost: No fee. Office on Aging Grant program. Donations accepted. Paterson only.

Medication Management

The program includes the initial assessment by an RN, computerized drug screenings, medication organizers, and weekly RN home visit to monitor medications.

Daily home health aide visits and medication dispensing devices can be arranged for additional cost if necessary.

Hours/availability: Monday through Friday

Patient Cost: $75.00 per nursing visit

(This service may be covered through a grant from the Passaic County Office on Aging.)

The MD2 Medication Dispensing devise can automatically dispense medications at predetermined times with voice prompts.  The device will call pre-selected phone numbers if meds are not taken.   Medication compliance can be monitored on the internet by family and health care professionals with proper authorization.  The MD2 rents for $90 per month including on-line support.  For more information click the image below.

Seasonal Cleaning Service

Intermittent household cleaning for frail elderly and disabled is provided, up to 4 times per year.

Hours/availability: Monday through Friday

Patient Cost: No fee, Passaic County Office on Aging grant. Donation accepted.

Eligibility: 60+ years of age, frail, disabled, Passaic County resident

Bath Service

Bed, tub bath or shower services are offered, as well as assistance with grooming and dressing and, if time permits, the changing of bed linen.

Maximum one hour stay.

Hours/availability: 7 days a week

House Call Groceries for Seniors (shopping)

A volunteer shopper goes to the home of a senior, picks up a shopping list, money and coupons, shops for groceries and prescriptions, delivers and can put away groceries.

Hours/availability: 7 days a week, call for service Monday through Friday

Patient Cost: no cost to clients

Eligibility: 60+ years of age, or disabled, who are unable to shop for themselves. Call to find out if volunteers are currently available in your town.
