Community Support for Caregivers
The Alzheimer’s Disease and related dementias program is funded through the Administration on Aging, Administration on Community Living. This free program is intended to provide supports and services to individuals living with dementia and their caregivers and to promote early screening for dementia in adults with intellectual or developmental disabilities. The program offers caregivers free training in a behavioral intervention program called REACH. Caregivers also may have a free companion aide assist with their loved one’s care so that the family caregiver may participate in the REACH program. This program is open to caregivers throughout New Jersey.
The REACH program is phone-based series of education sessions over 2 to 3 months that will focus on new behavior-management skills and strategies for caregiver self-care and caregiving challenges. In 2019, Home Care Options, a Home Care Agency located in Wayne, was 1 out of the 11 agencies chosen across the nation to receive the Alzheimer’s Disease Programs Initiative. The Alzheimer’s Disease Programs Initiative, also known as ADPI, is a program intended to increase dementia-related awareness, develop dementia-capability in communities and agencies, and provide in-home support services to caregivers. ADPI also uses evidence-based interventions to help meet the needs of those who support persons living with Alzheimer’s disease, related dementias, and developmental disabilities. Some activities and interventions include education and training programs, delivery of in-home supports through the Community REACH model, and linkage to community resources.

What is the Alzheimer’s Disease Program Initiative (ADPI)?
The Alzheimer’s disease and other Dementia Program Initiative (ADPI) is a program to increase dementia-related awareness, develop dementia-capability in communities and agencies, and provide in-home support services to caregivers.
What is REACH?
REACH is an individual behavioral intervention that helps caregivers with their current caregiving situations in ways that will make a positive difference for them and their loved ones. The REACH program was delivered face to face or via telephone by a program Coach in 4 sessions over a two to three-month period.
Is there any cost?
There are no cost or financial eligibility criteria for the ADPI or REACH services.
